📋 Recording Instructions
  • Use the Voice Memo app on your iPhone or Android — not WhatsApp
  • Find a quiet room — no background noise, no echo
  • Record for at least 3 minutes — 5 minutes gives noticeably better results
  • Speak naturally, as if explaining a treatment to a patient
  • Vary your pace slightly — don't read in a flat monotone
  • Save as M4A or MP3 and upload below

Your Photos

These are used to build your visual avatar. One excellent photo is all that's needed to begin.

📸 Photo Guidelines
  • Face the camera directly — no angled selfies
  • Good, even lighting — natural light or a ring light works well
  • Neutral or white background preferred
  • Clinical photo: scrubs, surgical cap, loupes around neck if you have them
  • Professional photo: smart attire, clean background
  • JPG or PNG, at least 1MB in size — phone camera quality is fine

Consent & Submit

I, the undersigned, consent to my voice recording and photographs being used by Dr Vivian Onu-Nzegbulem (Dental Amplify) to create an AI-generated digital clone for professional and educational purposes. I understand that:

• All AI-generated content using my likeness will carry a mandatory AI disclosure and my professional licence / registration number
• My files are stored securely, are never sold, and are shared only with the processors that produce my clone, who cannot reuse them
• My voice and images will never be used to train any other AI
• I may request a copy, withdraw my consent, or ask for my data to be deleted at any time by emailing vivian@dentalamplify.com
• This consent covers use within the Dental Amplify programme only

Processing is based on my explicit consent under the data-protection law that applies where I am (in the UK and EU, Article 6(1)(a) and Article 9(2)(a) GDPR).
